A medical leave of absence letter is a formal letter that is written by an employee to his or her employer to request needed time off work for medical reasons.

Look at published books out there now. Find a jacket you think reads well and see if you can use it as a guide for creating your own pitch.
Pitch Part II This part of the letter covers important things that would probaby sound funny if you squeezed it into the Pitch.
